Ethicon Endo-Surgery, Inc. (EES) purchases advertising and exhibit space at educational events. Requests for this type of support must be submitted through the on-line application. These opportunities are promotional and must be separated from educational grants. In those cases where educational and advertising/display support is requested two separate requests must be submitted.

Please follow these guidelines:

• All opportunities must be at fair market value and directly related to promotion of EES products or services.
• All requests must include a list of opportunities provided consistent for all sponsoring organizations.

All requests should include:

• Name and date of the event or activity.
• Cost of the exhibit space or advertising at the event.
• Description of the benefits or opportunities provided for promotional funds at each level of support.
• Federal Tax ID number of sponsoring organization.
• Name, address and a contact person.
• Name and address to send the payment or product.
• A sponsorship agreement must be signed prior to release of funds.
