Requests for product donations should be submitted through our website. To be considered and allow processing time, please submit your requests a minimum of six weeks prior to your event. All requests should include:

• Name of your organization.
• A 501(c) 3 organization.
• Name, title and phone number of a contact person.
• Requested dollar amount.
• Intent of the funding distribution.
• Date the donated product is needed.
• Name and shipping address for donated product.
• Products needed. Please specify them by their product code and in single instrument quantities. (Please see product list in the application for product codes or contact your Ethicon Endo-Surgery, Inc. (EES) representative)

Requesting organizations and EES acknowledge and agree that any donation or sponsorship shall not obligate the organization to purchase, use or recommend the use of EES products.

Submission of a grant guarantees neither approval of the request nor support for the entire amount requested.
